Seniors with diabetes often struggle with:

How Our Services Support Diabetes Care
Our care model is designed to meet clients where they are—offering three service types that match different needs, routines, and budgets. Whether support is needed occasionally, at structured times, or throughout the day, we build programs that align with the realities of managing diabetes.
On-Demand Care Services
On-Demand Care is ideal for clients who are mostly independent but need help from time to time. We can assist with transportation to medical appointments, pick up prescriptions, help with a grocery run, or drop in for medication reminders or safety check-ins. This is a great option for clients who don’t need daily care but want peace of mind knowing someone can step in when needed.

Targeted Care Services
Targeted Care is best for clients who need support at key times of day to manage diabetes more effectively. A caregiver might visit in the morning to help with medications, prepare a healthy breakfast, and go on a short walk—or return in the evening to assist with dinner and ensure blood sugar levels are on track. This helps maintain a consistent routine and avoids the gaps that can lead to complications.

Companion Care Services
Companion Care is designed for clients who require more continuous support. It’s ideal for those with more advanced diabetes, mobility limitations, or additional chronic conditions. A dedicated caregiver can assist throughout the day with blood sugar monitoring reminders, meal planning and prep, hydration, mobility support, and overall wellness. This option also provides added safety for clients who live alone or may not recognize early signs of concern.

Caregiver Training
All Tandem Care caregivers receive diabetes-specific training to help them understand the condition and recognize potential warning signs. They’re trained to encourage healthy habits, prepare meals that align with dietary needs, and report changes to the care coordination team. With our real-time client portal, families can review visit notes and stay in the loop—without having to manage every detail themselves.
