How Tandem Care Supports End-of-Life Clients and Families
We are not a hospice provider—but we work closely with hospice teams to ensure families receive full-spectrum support. Our dedicated care coordinators can refer families to trusted hospice partners when appropriate and help coordinate services across both teams. We step in to fill the gaps and ensure no one feels like they’re going through this alone.
On-Demand Care Services
On-Demand Care allows families to bring in additional help during difficult moments—whether it's for bathing, repositioning, running errands, or simply having someone sit with their loved one while they rest. This flexible option gives families the ability to call for help when they need it most, without committing to a daily schedule.

Targeted Care Services
Targeted Care is ideal for families who need help at specific times of day, such as morning bathing and dressing or evening support with dinner and repositioning. These scheduled visits provide structure, consistency, and peace of mind while allowing hospice to focus on the medical aspects of care.

Companion Care Services
Companion Care is best for clients who need longer hours of support throughout the day. A consistent caregiver can provide personal care, comfort, and a calm presence—while also assisting with household tasks and offering respite to family caregivers. This level of support is often essential during the final weeks or days, when families want to be present without managing everything alone.

Caregiver Training
All Tandem Care caregivers are trained to provide sensitive, respectful end-of-life care. While we do not offer clinical services, our team understands how to support clients and families emotionally and physically—offering calm, compassionate care through even the most difficult days. Our care coordinators remain closely involved, adjusting the care plan as needs change and ensuring communication across all providers remains seamless.
