Our flexible care model ensures families get the help they need—without overcommitting to care they don’t.

How Our Services Support Clients with CHF

Managing CHF is all about consistency—daily habits, scheduled check-ins, and reducing preventable risks. Our three levels of service give families the ability to choose just the right amount of help and scale up or down as needed.

On-Demand Care Services

On-Demand Care is ideal for short-term or occasional needs. A caregiver can help after a hospitalization, provide transportation to cardiology appointments, assist with grocery shopping to support low-sodium diets, or offer coverage when a family caregiver needs a break. This is a great way to avoid gaps in care without a weekly commitment.

Targeted Care Services

Targeted Care works well for clients who need help at set times of day to stay on track. A caregiver might come in the mornings to assist with medications, breakfast, and mobility support, or in the evenings to help with winding down. These visits can also include wellness check-ins and gentle encouragement to maintain activity levels and hydration goals within limits.

Companion Care Services

Companion Care is best for clients who need longer hours of support throughout the week—especially those with more advanced CHF or frequent fatigue. A dedicated caregiver can assist with daily routines, meal prep, mobility, and emotional support, ensuring that clients feel safe, comfortable, and never alone during more vulnerable moments.

Caregiver Training

Our caregivers receive training on CHF-related care practices, including how to support clients with fatigue, monitor changes in condition, and encourage adherence to dietary and medication routines. While we do not provide medical care, we stay alert to symptoms and communicate observations through our real-time portal, keeping the family and care team informed.
